Direct lung injury refers to damage to the lung tissue caused by an insult originating within the respiratory system, such as pneumonia, aspiration, or inhalation injury. It is clinically important because it directly impairs gas exchange and can rapidly progress to acute respiratory distress syndrome (ARDS), requiring prompt recognition and management. A common complication is severe hypoxaemia due to alveolar damage and inflammation.
