Non-radiographic axial spondyloarthritis is a chronic inflammatory arthritis affecting the spine and sacroiliac joints without visible damage on X-rays but with active inflammation detectable by MRI or clinical features. It matters because early diagnosis allows timely treatment to prevent progression to ankylosing spondylitis, reducing pain and disability. It is strongly associated with HLA-B27 positivity.
