Romanus lesion

A Romanus lesion is an inflammatory erosion at the anterior or posterior vertebral corners seen on imaging, indicative of active inflammation in spondyloarthritis. It reflects early structural damage to the vertebral body, aiding diagnosis and monitoring disease progression. This lesion is closely associated with ankylosing spondylitis and can precede syndesmophyte formation.
