Paradoxical pulse is an exaggerated decrease in systolic blood pressure of more than 10 mmHg during inspiration. It is clinically important as it indicates impaired cardiac filling or increased intrathoracic pressure, often seen in conditions affecting the heart’s ability to accommodate venous return. A key association is cardiac tamponade, where fluid accumulation restricts ventricular expansion, causing this characteristic pulse pattern.
